Promotion & Giveaway Rules

The HARTWINN GROUP & JAXSCO RX Amplify Network 2026 $500 Giveaway (“Promotion”) is sponsored and administered by:

HARTWINN GROUP, LLC and JAXSCO RX, LLC are each referred to in these Official Rules as a “Sponsor” and collectively as the “Sponsors.”

This Promotion is a chance-based sweepstakes. A winner will be selected randomly from all eligible entries received during the Promotion Period.

The Promotion is being offered as a replacement entry opportunity because a technical issue prevented the Sponsor from receiving information submitted through the QR code displayed at the Amplify Network 2026 conference.

Because the original QR-code system did not transmit entry information to the Sponsor, the Sponsor cannot retrieve or verify attempted QR-code submissions. Any eligible attendee—including an attendee who did or did not attempt to use the original QR code—may enter through the corrected entry method described in these Official Rules.

The Promotion is open only to individuals who, at the time of entry:

• Are legal residents of one of the 50 United States or the District of Columbia;
• Have reached both 18 years of age and the legal age of majority in their state or district of residence;
• Personally attended the Amplify Network 2026 conference in Indianapolis, Indiana; and
• Satisfy all other requirements stated in these Official Rules.

The Sponsor may request reasonable proof of conference attendance, such as an event registration confirmation, conference badge, or other reasonably reliable documentation.

Employees, officers, directors, contractors, and representatives of the Sponsor, HARTWINN GROUP, their affiliated or subsidiary entities, and any advertising, promotional, fulfillment, or administrative agencies involved with the Promotion are not eligible.

Immediate family members and household members of excluded persons are also not eligible. “Immediate family” means a spouse, parent, child, sibling, domestic partner, and each such person’s spouse, regardless of where they reside. “Household member” means any person who has shared the same residence with an excluded person for at least three months during the twelve months preceding the beginning of the Promotion.

The Promotion is void outside the 50 United States and District of Columbia and wherever prohibited or restricted by law.

The Promotion begins on August 3, 2026, at 6:00 PM Central Time and ends on August 14, 2026, at 3:30 PM Central Time (“Promotion Period”).

The Sponsor’s email system and computer records are the official timekeeping devices for the Promotion. Entries received before or after the Promotion Period are not eligible.

During the Promotion Period, send an email to: info@jaxscorx.com.

Use the following subject line:

The email must include:

• Entrant’s full legal name;
• Entrant’s email address;
• Entrant’s state of legal residence;
• Entrant’s organization and professional title;
• Telephone number; and
• A response to: “Tell us about your 340B program needs.”

A purchase, payment, donation, sales inquiry, marketing consent, or existing or prospective business relationship with the Sponsor is not required to enter or win.

An entrant who attempted to submit information through the original QR code must submit a new email entry during the Promotion Period because the Sponsor did not receive the original QR-code submission.

Normal internet-access or email-provider charges imposed by the entrant’s provider, if any, are the entrant’s responsibility.

Limit one entry per person and one entry per email address during the Promotion Period.

If more than one entry is received from the same person or email address, only the first complete and otherwise eligible entry received will be considered.

Entries generated by scripts, bots, macros, automated systems, artificial identities, or other automated means are prohibited. Entrants may not submit an entry on another person’s behalf.

Entries that are incomplete, illegible, fraudulent, materially inaccurate, submitted outside the Promotion Period, or otherwise fail to comply with these Official Rules may be disqualified.

The Sponsor will not arbitrarily reject an otherwise valid entry.

On or about August 14, 2026, at 4:00 PM Central Time, the Sponsor or its designated administrator will select one potential winner through a random drawing from all eligible entries received during the Promotion Period.

The random drawing should be documented by retaining:

• The final eligible-entry list;
• The date and time of the drawing;
• The randomization method used;
• The selected entry; and
• The names of at least two individuals who witnessed or verified the drawing.

The entrant does not have to be present to win.

The odds of winning depend on the number of eligible entries received.

The person initially selected is only a potential winner until the Sponsor verifies eligibility and compliance with these Official Rules.

One prize package will be awarded in connection with one randomly selected eligible entry. Subject to verification:

The total approximate retail value of the prize package is $600.

The Organization Prize will be paid directly to the identified organization and will not be paid to or through the individual winner. The individual winner has no personal ownership interest in the Organization Prize and may not exchange, transfer, or redirect it to another organization except as expressly permitted by the Sponsor under these Official Rules.

For purposes of awarding the Organization Prize, the recipient will be the organization identified in the winning entry when it was submitted, regardless of whether the individual winner’s employment or affiliation later changes. The organization must be a legally established United States entity, must be legally permitted to accept the prize, and may be required to provide a completed Form W-9 and confirmation from an authorized organizational representative.

The individual winner and organizational recipient are separately responsible for any federal, state, or local taxes resulting from their respective prizes. Prizes and awards are generally taxable to the recipient under federal tax rules.

Each prize will be paid directly to its respective verified recipient. The $100 Individual Prize will be paid to the verified Individual Winner. The $500 Organization Prize will be paid directly to the verified Organization Recipient and will not be paid to, transferred through, or redirected by the Individual Winner.

Payment will be made by company check, ACH transfer, credit card, or another cash-equivalent payment method selected by the Sponsors. Each prize will be delivered within ten (10) business days after the applicable recipient has been verified and has submitted all reasonably required documentation, tax forms, payment instructions, and other information necessary to process the payment.

The right to receive a prize may not be transferred or assigned before payment. No prize substitution is permitted, except as required by applicable law or as otherwise expressly provided in these Official Rules.

No interest will accrue or be paid on either prize.

The potential winner will be contacted using the email address and, if voluntarily provided, telephone number included with the entry.

The Sponsor will attempt to notify the potential winner within one business day after the drawing.

The potential winner must respond within 72 hours after the Sponsor sends the first notification. The Sponsor may require the potential winner to:

• Confirm the information provided with the entry;
• Provide reasonable proof of identity, age, residence, and conference attendance;
• Sign an affidavit or declaration of eligibility;
• Sign a liability release where lawful;
• Complete any legally required tax documentation; and
• Sign a separate publicity authorization if the Sponsor wishes to use more information than permitted under Section 11.

If the potential winner:

• Cannot be contacted;
• Does not respond within 72 hours;
• Refuses the prize;
• Fails to provide required verification;
• Is determined to be ineligible; or
• Fails to comply with these Official Rules,

the potential winner will be disqualified, and the Sponsor will randomly select an alternate potential winner from the remaining eligible entries.

The Sponsor will continue selecting alternates until the prize is awarded, provided at least one eligible and responsive entrant remains.

The winner (organization and individual) is solely responsible for all federal, state, and local taxes and any other expenses associated with accepting or using the prize that are not expressly included in these Official Rules.

The Sponsor may request tax or identification information when required by applicable law.

Cash prizes generally constitute taxable income to the recipient even when the prize is below the threshold at which the payer is ordinarily required to issue Form 1099-MISC. The current federal Form 1099-MISC reporting threshold for prizes and awards is generally $600, but the winner remains responsible for reporting taxable income regardless of whether a form is issued.

After the winner has been verified, the Sponsor may publicly announce the winner’s:

• First name;
• Last initial;
• Organization (Covered Entity) affiliation;
• Professional Title; and
• City and state of residence.

The Sponsor will not publish the winner’s street address, telephone number, email address, or other sensitive contact information.

Except where prohibited by law, acceptance of the prize constitutes permission for the Sponsor to use the winner’s name, first-name-and-last-initial identification, city and state, photograph, likeness, voice, and brief statements concerning the Promotion for reasonable promotional and informational purposes without additional compensation.

The Sponsor will collect and use entry information for the following purposes:

• Receiving and recording entries;
• Determining eligibility;
• Administering the random drawing;
• Contacting and verifying the potential winner;
• Delivering the prize;
• Preventing fraud or duplicate entries;
• Maintaining legally or operationally necessary records; and
• Complying with applicable legal, accounting, and tax requirements.

Entry into the Promotion authorizes the Sponsor to send communications reasonably necessary to administer the Promotion. Entry does not by itself constitute consent to receive unrelated marketing emails, automated telephone calls, or text messages.

Any consent to receive promotional or marketing communications must be obtained separately, must be optional, and must not affect eligibility or the entrant’s chances of winning.

The Sponsor may share entry information with employees, affiliates, professional advisers, and vendors only to the extent reasonably necessary to administer the Promotion, provide the prize, maintain security, or comply with law.

The Sponsor will not sell entry information to unrelated third parties.

Non-winner entry records should be retained only for as long as reasonably necessary to administer and document the Promotion, recommended as 12 months after the Promotion ends, unless a longer period is required by law. Winner, tax, and accounting records may be retained longer.

By entering, each entrant agrees to:

• Be bound by these Official Rules;
• Be bound by the Sponsor’s good-faith decisions concerning the administration of the Promotion;
• Provide truthful and complete eligibility information; and
• Accept the results of the random drawing, subject to the verification process.

The Sponsor may disqualify an entrant who tampers with the entry process, submits fraudulent information, attempts to obtain more than the permitted number of entries, interferes with another person’s participation, or violates these Official Rules.

If fraud, malware, unauthorized intervention, a platform failure, a widespread communications outage, or another circumstance outside the Sponsor’s reasonable control materially compromises the fairness or integrity of the Promotion, the Sponsor may suspend, cancel, extend, or modify the Promotion.

If the Promotion is terminated early, the Sponsor may conduct the random drawing from eligible entries properly received before the termination, provided doing so remains fair and lawful.

Any material modification will be posted promptly on the same webpage as these Official Rules and, when reasonably possible, on the social-media channels used to advertise the Promotion.

To the fullest extent permitted by law, entrants agree to release and hold harmless the Sponsor and its parent companies, affiliates, subsidiaries, officers, directors, employees, agents, contractors, and promotion administrators from claims arising directly out of:

• Participation in the Promotion;
• Acceptance, possession, use, or misuse of the prize;
• An entrant’s violation of these Official Rules; or
• An entrant’s fraudulent or unauthorized conduct.

This release does not waive claims that cannot lawfully be waived, including claims arising from gross negligence, willful misconduct, or other liability that applicable law prohibits the Sponsor from disclaiming.

The Sponsor is not responsible for entries that are delayed, misdirected, incomplete, blocked by email filters, corrupted, or not received because of technical problems outside the Sponsor’s reasonable control.

Nothing in this section eliminates the Sponsor’s responsibility to administer the corrected entry process fairly after learning that the original QR-code system did not transmit submissions.

This Promotion is sponsored and administered solely by HARTWINN GROUP, LLC and JAXSCO RX, LLC. It is not sponsored, endorsed, administered by, or associated with Facebook, Instagram, Meta, LinkedIn, X, or any other social-media platform.

These Official Rules will be governed by the laws of the State of Tennessee. Any legal action arising from the Promotion must be brought in a court of competent jurisdiction located in Williamson County or Davidson County, Tennessee.

The verified winner’s first name, last initial, city, and state may be requested by emailing: info@jaxscorx.com. Use the subject line: AMPLIFY 2026 GIVEAWAY WINNER LIST REQUEST. Requests must be received no later than 60 days after the Promotion ends.

If any provision of these Official Rules is found invalid or unenforceable, the remaining provisions will remain in effect.
